To receive your rebate, follow these steps:

1. Call Martens Farms to purchase any of the qualifying EZ Products at regular retail prices by June 30, 2009.
   
   
2. Go to http://agleaderEZ-products.apfco.com/w1328/index.cfm and complete the required rebate information online.
   
   
3. Print and enclose that completed rebate form and the following proof of purchase for your EZ Product(s)

   a. Copy of your sales receipt or invoice from Martens Farms
   b. Box label including product serial number and part number
   
   

4. Mail these to the address on the rebate form on or before July 30, 2009.
   
   
5. Payments are made directly to the customer within 8 weeks.
